is gcd missing from gmpxx.h? no, but the include file never got installed

Don't you think its amazing that nobody ran into this basic overdight in
20 years?

As an intermediate workaround, please pass --enable-cxx to configure.
That also has the advantage that you'll get libgmpxx.{a,so} compiled.
